Jasmin Sanchez
Hello, my name is Jasmin Sanchez, ACSW, and I currently serve as a Behavioral Health Care Manager at Astrana Health in the ECM program, specifically in the IEHP team. I hold a Master of Social Work from the University of Texas at Arlington (2022) but initially began my graduate studies at USC prior to relocating to Texas to attend UTA.
I bring a diverse background in serving vulnerable and underserved populations, including homeless youth, survivors of domestic violence, foster care youth and minors, unaccompanied minors in ORR, and incarcerated male adults. My experience spans from street outreach, housing placement through transitional/rapid-rehousing programs, case management, crisis intervention, and conducting individual therapy as an Associate Clinical Social Worker.
Notably, I provided mental health services at the Los Angeles County Twin Towers Correctional Facility, where I facilitated group therapy for incarcerated adult males identified as high-risk for self-harm which included anywhere from 4-20 male adult participants. This role deepened my understanding of trauma-informed care in a correctional setting.
I’ve also worked alongside DCFS, ORR, and community-based organizations such as foster care agencies and domestic violence shelters to support minors and families through emergency removals, reunifications, housing, and mental health interventions.
At Astrana, my focus is on ensuring underserved and minority populations receive accessible and appropriate healthcare through Enhanced Care Management (ECM).
In my free time, I enjoy spending time with my brothers, cousins, and other family members, including my husband. I also love outdoor activities, road trips, and learning about different cultures through food, history, and shared experiences.
