Kate Acosta

Hi, my name is Kate Acosta. I am a Licensed Vocational Nurse (LVN) and currently serve as a Lead Care Manager for the Enhanced Care Management (ECM) Program. I graduated from nursing school in 2024 and began my career in home health case management, where I supported patients and families with care coordination and education in the home setting. Prior to becoming a nurse, I gained several years of experience working in Applied Behavior Analysis (ABA) therapy and at a school for children with physical disabilities and special needs, where I worked closely with LVNs, teachers, and a registered nurse in a classroom setting. Since April, I have been part of the ECM team, where I help Medi-Cal members with complex medical, behavioral, and social needs access the resources, care, and support they deserve. 

I am especially passionate about helping members in underserved communities, where language, cultural, and systemic barriers can make it difficult to navigate healthcare and social services. Many people don’t know what resources are available, and I find it meaningful to advocate for those who may feel hesitant or afraid to speak up for themselves. My belief is that care management is not just about completing tasks—it’s about truly caring for people, listening to their needs, and walking alongside them in their journey. Outside of work, I enjoy spending time with my family, reading, and relaxing by watching movies and shows.
