Samantha Byers

Hi I am Samantha.  I am a dedicated and experienced Licensed Vocational Nurse (LVN) who has been providing compassionate, patient-centered care since 2014. With a career spanning over a decade of experience in wound care, medication teaching, and care coordination across skilled nursing, home health, and Enhanced Care Management (ECM). I’m passionate about advocating for health equity and ensuring every patient feels seen, supported, and respected. 

Currently, I serve as a Lead Care Manager in the Enhanced Care Management (ECM) Program, where I support Medi-Cal members with complex medical, mental health, and social needs. I assist clients in navigating the healthcare system, managing chronic conditions, accessing specialty care, and overcoming barriers to wellness such as housing, transportation, or food insecurity.  My approach is rooted in advocacy, empathy, and whole-person care—empowering patients to achieve stability and improved health outcomes. I’m passionate about health equity and ensuring that every patient I care for feels seen, supported, and respected throughout their journey. Beyond my clinical role, I enjoy spending time with my family, going on adventures with my husband and our dog, visiting Disneyland, cooking, and working on fun DIY projects around the house.
