Susana Hernandez

My name is Susana Hernandez, Lead Care Manager for the Enhanced Care Management Program. With over 10 years of experience working and advocating for vulnerable and oppressed populations and communities. Such as individuals with disabilities, chronic medical conditions, mental health disorders, victims of crime, substance use and homelessness. I am passionate about making a difference and helping those in need, whether it’s by providing care coordination, crisis intervention, advocating, educating or providing community resources. Always striving to become the best version of myself and I believe that every challenge or setback presented is an opportunity to learn and grow. Outside of work, I enjoy cooking, decorating my home, and spending time with my kids.
